Payment in full expected at the time of service. If you are a member of one of the following insurances companies, we will be happy to bill your insurance company for you. Please bring with you to your appointment:

1. Your health insurance card
2. Your doctor’s written orders, if they were given to you
3. Insurance referrals, if needed
4. Patient information form (filled out)
5. Hearing history form (filled out)

(Applicable co-payment will be collected at the time of service.)
